Application Process
Effective January 1, 2018, Forest County has established a Uniform Parcel Identifier (UPI) System to streamline property transactions.

steps to Obtain a UPI:

***If any of the below criteria are not met, the UPI request will be denied. No additional fees will be charged until all required information is provided. UPI numbers will be assigned within three (3) business days after all required documentation & fees are submitted and will then be forwarded to the Recorder's Office for recording.***

  1. Fill out UPI Application Linked below:
    1. Forest County Uniform Parcel Identifier Application
      • Complete Application: Include all required information
      • Certificate of Provision of Information: Petitioners must sign this to complete the application.
      • Subdivision Plans (if applicable): If the deed conveys only a portion of the currently assessed parcel, an approved subdivision plan survey map number must accompany the submission.
  2. Pre-Approval Process:
    1. To avoid delays, please email ALL required documents for pre-approval before submitting them. This ensures compliance with the requirements and helps streamline the process.
      • A valid parcel description showing metes and bounds must match the previous deed and be included in the document to be recorded.
      • A Map # (e.g., xx-xxx-xxx-xx) must be included in the description.
      • The 911 Address must be included in the description. ONLY the street address should be provided—do not include the city, state, or zip code.
        • If it is Vacant Land, label it as such.
    2.  All documents can be emailed to dzofcin@forestcountypa.gov for pre-approval.
  3. Post-Approval:
    1. A $20.00 fee is required for documents needing a UPI. 
      •  Please make the check payable to Forest County for the UPI fee.
      • For all other documents, please make checks payable to Forest County Recorder of Deeds.
    2.  Completed documents and UPI applications can be submitted between 8:00 AM and 3:00 PM,  Monday through Friday.
      • Documents received after 3:00 PM will be processed the next business day.
    3.  All documents, including checks for both the UPI fee and recording fees, can be mailed or delivered together.
      • Please include your UPI application form and a self-addressed stamped envelope so we can send you your recorded copies.

The following documents require a UPI:

***A valid parcel description showing metes and bounds
must match the previous recorded document and be included in the document to be recorded.***

  • A Deed or other instrument that transfers any interest in specific real estate, including Quitclaim Deeds, Fiduciary Deeds, Corrective Deeds that correct clerical errors in a prior deed or deeds.
    • Mineral Deeds do NOT require a UPI but DO require a valid parcel description showing metes and bounds.
  • Declaration of Taking by Condemnation.
  • Installment Land Contract (Articles of Agreement).
  • Adverse Possession Affidavit.
  • Affidavit Affecting Title to Real Estate.
  • Easements DO require a UPI but do NOT require a valid parcel description showing metes and bounds.
  • Right-of-Way.
  • Ground Lease or other Lease Agreement, including a Memorandum of Lease, to the extent the lease involves or affects any interest in specific real estate.
  • Right of First Refusal, to the extent it involves or affects any interest in specific real estate.
  • Option to Purchase, to the extent it involves or affects any interest in specific real estate.
  • Quiet Title Decrees.
  • Mortgage, Assignment of Mortgage, Release/Partial Release of Mortgage, Extension, Postponement, Subordination of Mortgage, Open End Mortgage, Assignment of Rents, or other secured transaction to the extent it involves or affects any interest in specific real estate.
  • Distribution Decree, Bankruptcy Notice, or other court order affecting specific real estate.
  • Holding Tank Agreements
  • Recreational Cabin Affidavits (RCA)
